Here’s your chance. The National Institutes of Health is conducting a survey that can be found on the front page of the American Chronic Pain Association. The Chronic Pain Survey is meant to help doctors understand better life with chronic pain. This is part of what is written:
You will be asked some general questions about your social and medical history, and then you will complete a set of pain and quality of life (QOL) questionnaires. You will be asked to complete 3 QOL questionnaires on the internet via a personal computer. The QOL topics to be examined throughout this study are pain, emotional distress, fatigue, physical functioning, and social well-being. The length of time to complete the questions should take about 30 minutes.

This study will allow doctors to learn more about the quality of life of people with pain. There are no direct benefits that result from participation in this study, except that answering the questions might help you to think about things related to your health and treatment that may be important to you. The questions may also be interesting to answer. Your participation in this study will help researchers and clinicians make patient quality of life easier to incorporate into treatment for pain.
